| All first time members need to fill this form and submit with 150 baht. You will then get assigned a house color. | |

| Table Tennis | A 70 baht registration fees for this event. |
| Bowling | A 250 baht registration fee for this event that includes 3 games, free soda pop, and free shoes. |
| Golf | A 2,200 baht registration fee for this event is needed. |
| Indoor Soccer App. Information | This is not an application form, but application information. Applications must be completed on-line for this event. Applications must be filed online at www.thaisikh.org/YTSA/Soccer.htm. |
| Basketball Application Form | Please fill application form and submit to a captain, event organizer, or organizer with 500 baht. |
| Badminton Application Form | Please fill application from your house captain or event organizer or YTSA organizer. |
