THIS MEMBERSHIP ONLINE APPLICATION FORM SHOULD BE USED ONLY IF YOU WISH TO BECOME A MEMBER WITHOUT IMMEDIATELY PARTICIPATING IN ANY PAID EVENT. IF YOU WISH TO APPLY FOR MEMBERSHIP TO IMMEDIATELY PARTICIPATE IN AN EVENT SUCH AS SOCCER, YOU SHOULD APPLY FOR MEMBERSHIP AT THE EVENT WEBSITE (SUCH AS SOCCER WEBSITE), SO THAT YOU ONLY FILL ONE FORM. 

FILL THIS APPLICATION FORM ONLY AFTER MAKING 150 baht PAYMENT FOR MEMBERSHIP and RECEIVING A PIN NUMBER ON THE COUPON.  COUPONS CAN BE PURCHASED FROM AN ORGANIZER, AN ADMINISTRATIVE STAFF OF GURDWARA, OR A HOUSE CAPTAIN.

Instructions:

  1. Complete ALL the information that are in red color such as First name, Last name, Birth Date.  They cannot be left blank.

  2. The required membership payment coupon number field is exactly 13 characters such as in M323233223323 (get after paying 150 baht).

  3. The Birth Date day must have value between 1-31, month between 1-12, and year 4-digits between 1920-2004.
